This is an intake virtual/telephone appointment with one of our wonderful office staff to ensure we have accurate and complete information about you. It should take about 10 to 20 minutes.
​What to expect now?

Now that you have scheduled your New Patient Registration appointment, you will receive an email with patient forms to complete prior to the appointment on demographics, medical history and social determinants of health.

During this appointment, one of our wonderful office staff will call you for the appointment to ensure we have accurate and complete information about you. It should take about 10 to 20 minutes.

We will make sure you are familiar with our office location.
​

We will provide you a tentative office visit with one of our providers/physicians, while we ensure we have all of your clinical records.
